The undersigned ("Cosigner") hereby submits this application to co-sign an apartment lease for applicant named above ("Applicant") to induce D&D Partners Trusted Choice LLC, an Illinois Limited Liability Company ("D&D Partners") to process Cosigner's application and any landlord to whom Applicant is submitted as a potential tenant to enter into a lease with Applicant and Cosigner. Cosigner represents and warrants that: (a) all of the above information, and any other documents or information provided, is true and accurate; and (b) Cosigner has no adverse credit history. In the event any of the Cosigner's representations and warranties are untrue and inaccurate in any way, any deposit paid by Applicant or Cosigner shall be paid to D&D Partners as liquidated damages for such untrue or inaccurate representations or warranties and no funds shall be refunded to Applicant or Cosigner. Cosigner hereby deposits with D&D Partners the amount of $ to be applied, if the Applicant's application is accepted, towards Applicant's first month's rent. If Applicant application is accepted, Applicant and Cosigner shall promptly sign the lease presented by landlord. If Applicant's application is rejected, the deposit shall be returned unless it is paid to D&D Partners for the reasons described above. Cosigner is also required to pay a $70.00 co-signer fee for D&D Partners to perform a credit investigation.
